Insurance Accepted

Fidelity Health accepts most health insurance plans. As much as possible, we will keep the service rates of our home health programs as affordable as can be. We believe that health care should be accessible to all.

These payment options are available to clients:

Many insurance plans are now considered “tiered” plans, and have different levels of costs depending on where you go for care. It is important that you check with your own health insurance plan to see what level of coverage you will receive at a specific agency. Even if you see your insurance plan listed, additional referrals or authorizations may be required.  Some services may not be covered by your insurance coverage at every location. The level of coverage provided to you is determined by your insurer. Please contact your health insurance plan or employer for your specific questions.

Important things to know:

Your insurance plan may not cover all services Fidelity Home Health provides, including diagnostic tests and behavioral health services. Our physicians and staff may not participate in (or be “enrolled” in) all of our accepted insurance plans. Always check first with your insurance provider.
